Please remember to include your name, date of birth, the name of each of the medications requiring a refill, and the pharmacy you wish to use.
Pain Management of Tulsa is contracted with most major insurance and Workers' Compensation carriers to provide care to our patients on an in-network basis. Our list of carriers is not compressive. When you schedule your appointment, our staff will use your insurance information to verify your insurance coverage.
